Access Agencies and The Connecticut Home Care Program

Program Description

Attendees will learn about that State’s rebalancing efforts and the coordination of transition and diversion services. Attendees will leave with a better understanding of the roles and responsibilities assigned to various State departments and contractors with a goal of reducing the complexity and knowing where to turn for assistance if a client is stuck at any point on the continuum. Additional learning objectives include, clarity around the Universal Assessment tool and how its design is used in care plan development; direct services available through the waiver with a focus on new offerings such as Adult Family Living; and how Community Choice Medicaid benefit works and who is eligible.

Biographies

Gayle P. Kataja, RNC, MS
Connecticut Community Care, Inc.

Gayle Kataja is the Director of Quality and Private Initiatives for Connecticut Community Care, Inc. (CCCI).  CCCI is a private non-profit, statewide agency providing care management to older adults.  She holds a bachelor’s degree in nursing (Russell Sage College) and a master’s degree in health care administration (Rensselaer).  She is certified by the American Nurses’ Association in Gerontological Nursing. She was the recipient of the National League for Nursing’s Centennial Leadership Award.  She is a member of the Medical and Scientific Advisory Council of the Alzheimer’s Association, Jefferson House Community Collaborative, the Connecticut Nurses Association, America Nurses Association and Tunxis Community College, Human Services Advisory Committee.

Ms. Kataja lectures extensively on care giving issues, long term care, legal and ethical issues, Alzheimer's Disease, and long-term services and supports for older adults and persons with disabilities.  She has published articles on respite care, ethics and case management in long-term care.  She has provided consultation to numerous entities defining the role of care management with emphasis on social determinants of care, and care transitions.

Marie Allen is the Executive Director of the Southwestern CT Agency on Aging (SWCAA) promoting independence from Stratford to Greenwich Connecticut.  She holds a Masters of Business Administration. Building on the strength of the aging provider network, SWCAA coordinates services for 3,000 southwestern Connecticut residents providing an alternative to nursing home placement.  Marie serves on Connecticut’s Long Term Care Advisory Council, she is the public advocacy coordinator for the CT Association of Area Agencies, an advisory member of Yale Hospital’s Connecticut Older Adult Collaboration for Health (COACH), UCONN’s Care of Persons with Dementia in their Environments (COPE) and the Connecticut Medicaid Assistance Program Council.
